Fall Sports Form Information:
Sports physicals for Fall 2025 athletics will be accepted from May 20, 2025 through June 17, 2025.
Please note that parent forms must not be signed before May 20, 2025—any forms dated prior will not be accepted.
All student-athletes must have a completed and approved physical on file before tryouts begin in August to be eligible to participate.
Important Note for Cheer and Dance Teams:
Due to tryouts taking place during the last week of May, physical forms for cheerleaders and dancers must be submitted by May 12, 2025.
All necessary forms can be found in the Athletic Handbook.
