Spring Sports Form Information 2026:

Sports physicals for Spring 2025/2026 athletics will be accepted from:
December 16, 2025 – February 6, 2026

Important:

Parent/guardian signatures on parent forms must be dated on or after December 16, 2025.
Forms signed before this date will not be accepted.
All student-athletes must have a completed and approved physical form on file before tryouts begin in March to be eligible to participate.
Be sure to submit your completed forms to your appropriate grade-level nurse’s office or place them in the drop box located outside the main entrance of the high school.

9th & 10th Grade: Annex Nurses Office
11th & 12th Grade: Main Nurses Office

All necessary forms can be found in the Athletic Handbook.

Athletic Event Spectator Policy

GENESIS SPORTS REQUIREMENT

In order to be eligible for participation in athletics, the Sports Interest Form must be completed in Genesis.
Please find the Sports Interest Form by doing the following steps:
*For Fall Sports only Sports Interest form will be available mid July.

Sports forms are being accepted starting May 20, 2025 and are due no later than June 17, 2025 with parent forms signed no earlier than May 20, 2025.	Sports forms are being accepted starting September 10, 2025 and are due no later than October 24, 2025 with parent forms signed no earlier than September 10, 2025.	Sports forms are being accepted starting December 22, 2025 and are due no later than February 6, 2026 with parent forms signed no earlier than December 16, 2025.

SBHS ATHLETICS now has the GoFan digital ticketing option for home football games this season! Cash tickets CAN still be purchased at the ticket booth each game, but if you want to purchase tickets in advance, you can get your GoFan tickets here!
